Physician services
Care Plans are collaboratively developed by focusing on goals of life. They are revised every several months, depending on the needs and interests of the client and their loved ones.
The first 8 weeks typically includes several home visits, medical records review, communication with health care providers when needed, managing medications, correspondence, ordering durable medical equipment and coordinating consultations with other care providers as needed.
Text and phone communication between home visits allows us to streamline the care process in a way that respects the time and energy of the client, family and lay care providers. Subsequent months typically require 1-3 hours of physician time depending on patient and family needs and medical complexity.
Supportive Services
Other professional support (ie: psychotherapist, social worker, medical equipment) may be offered as needed). When possible, we help our clients optimize benefits for additional care from their existing healthcare financial resources.

Support implementing doctor's orders and advice in the home between house calls and visits with doctors. Coordinating private care needs. Additional support with medication management and medical equipment as needed.

What is the cost?
We offer straightforward pricing for our services. No deductibles or retainers. Because of our extensive network we know how to find the most valuable services and medications at the best cost and will always work to provide cost savings to our members.
Our services are billed hourly and can include the physician's travel time when conducting home visits.
How is it billed?
These services are not reimbursable by insurance, Medicare, or Medicaid.
Invoices are sent on the 15th and 30th of the month and are payable within 5 days of receipt by check, Zelle, PayPal, credit, or debit card.
