How is this different than hospice care?

Hospice Care is an option when a patient with a life limiting illness has reached a point at which their life expectancy (as measured by clinical disease trajectory and functional capacity) is predicted to be 6 or less months.


This offering is a patient and family centered care model that holds the patient and their loved ones in the center of our focus. Many things can be handled by a phone, text or a house call from physician who is knows the patient and their family and is available 24/7.

At such time as hospice care may be appropriate, we continue to remain involved in your care.
